EAC Adoption Story Submission Form
 To submit your adoption story, please use the form below to upload your story and photos (.zip compressed format recommended) filling in the appropriate information. Do not leave any blank text fields. After completing the form, click the "submit" button at the bottom to send the document.
We would appreciate a set of "before/after" photos, along with your story, showing your child in the orphanage or at the time of their adoption and a recent photo of your child now.
Form Tips:
• Click 'browse' and choose files you wish to send, then press 'upload' to send them.
• Multiple files can be submitted by repeating the upload process
• Please remember to name your files appropriately so they can be identified (ex. LASTNAME-1.jpg)
• Compressing your file(s) in .zip format is recommended
• Following file types accepted: .zip, .jpg, .jpeg, .doc, .docx
